Go to our Account Login page and click the "Get a card" button to complete the online form.
Visit any Santa Clara County Library within 30 days and provide your date of birth along with a valid form of identification showing your picture, name, and proof of your current address to complete your registration and receive your card. (Acceptable forms of proof of address)
